After the repairs are completed, they must submit a reimbursement request with a note explaining the reimbursement request and an original paid repair invoice. The repair invoice must include the customer's name and address, vehicle identification number (VIN), authorization number obtained from Mopar’s office, repair date, vehicle mileage on the odometer at the time of repair, description of services performed, and labor and parts costs.
